Exercise your data rights
Use this form to request access, rectification, erasure and the other rights under Articles 15-22 of the GDPR. The request is sent to the Data Protection Officer (DPO).

Fill in the fields. The request goes to dpo@clinicaunica.it: the Controller responds within 30 days (extendable by 60 in complex cases). We may contact you to verify your identity.

You can also exercise your rights by writing directly to dpo@clinicaunica.it or via certified e-mail (PEC) to clinicaunica@pec.it. If you believe the processing infringes the GDPR, you may lodge a complaint with the Italian Data Protection Authority.
